All you need is a computer (a SKYPE account) or a phone.
During an online consultation, you will have the opportunity to present your health concerns to Dr. Kosmides using the latest in video technology or by telephone. During the consultation, you will relate your health history, the symptoms of your presenting health concern and the discuss results of any existing laboratory or diagnostic reports.
Dr. Kosmides will suggest appropriate therapeutic options in accordance with the scope of practice. These may include nutritional and lifestyle modifications, whole food supplementation and food recommendations.

Step 2 - Download & Print Form Required
Dr. George Kosmides' office offers our patient form(s) online so they can be completed in the convenience of your own home or office.
- If you do not already have AdobeReader® installed on your computer, Click Here to download.
- Download the necessary form(s), print them out and fill in the required information.
- Fax us your printed and completed form(s).
New Patient Health History Form - Required

Please send your Skype Account information to Dr Kosmides in advance of your appointment.
Payment Policy
All sessions must be paid in advance or just prior to the appointment.
The day of your online medical consultation, please be on time. Doctor Kosmides will contact you from his computer system.
Cancellation Policies
To ensure that your scheduled appointment time is reserved for you, we ask that our 24-hour cancellation policy be respected. Failure to notify the office within this time frame is subject to a $30.00 cancellation fee.
